This Dog Toy is based on weight. We recommend choosing the size that is suitable for your dog's weight. The Small is suitable for puppies up to 35 pounds and the Medium/Large is suitable for puppies between 30-65 pounds.

Does the hole go through the ball so you could put a rope through it or a string?
