Do I need to remove the fish and plants and all the water to use this, or does it just get added into the water already in the tank?

Plants and fish do not have to be removed in order to use this product. However Re-Fresh may be harmful to snails and shrimp. Do not overdose on tanks containing these organisms. Wait 5 to 7 days after final dosing to add these organisms.
