Is this good for surgery recovery?

These were not designed for post-surgery use. They are intended as comfortable pajamas to help keep pets warm and cozy.

How does this stay fastened? With velcro?

The pajamas do not have any fasteners. They are pulled over the head first and then the legs are put in.

Are they true to size or shrink if washing

These are true to size. The washing instructions are as follows: Turn inside out, machine wash cold with like colors on gentle cycle. Only non-chlorine bleach when needed. Tumble dry low.
