Chewy Vet Care Membership Plan Terms and Conditions Effective Date: October 15, 2025

By making an initial payment or using a Membership Plan from a Chewy Vet Care practice, you agree to these Membership Plan terms and conditions (the “Terms”) governing membership plans (the “Plan”) offered by Chewy Vet Care and/or our affiliates (“we,” “us,” or “our”) to the designated dog or cat patient (your “Pet”) and the person who owns the Pet and/or pays for and manages the Plan (“you” or “your”). These Terms supplement the terms and conditions set forth in the Chewy Terms of Use (including the arbitration agreement and dispute resolution proceedings), Chewy Vet Care Terms of Use, Chewy Privacy Policy, and all other applicable rules, policies, terms, limitations and requirements provided on the Chewy Vet Care website or in your Chewy Vet Care Practice, all of which are incorporated into these Terms. Please read all Terms carefully. Any changes we make to this agreement will be effective as of the date posted unless otherwise noted.

EACH PET REQUIRES THEIR OWN PLAN

The Plan cannot be transferred to other pets or new pet parents (for example, in the case of adoption or rehoming). The Plan only applies to the individual Pet owned by you while the Plan is active. You can use the Plan Benefits (as defined below) at any Chewy Vet Care location. You can use your Plan at any Chewy Vet Care practice. The Pet may only receive medical services or benefits from the Plan at a Chewy Vet Care practice.

PLAN DETAILS

The Plan includes services and benefits identified in the list for your applicable Plan ("Plan Benefits"). The Plan does not cover services or benefits from other providers, or fees for services recommended due to illness or injury, unless listed in the Plan Benefits. We will provide services according to standard veterinary care and applicable law. It is your responsibility to make appointments and ensure that your Pet visits us for exams.

With an active Plan, exam fees are waived as long as they are for routine care, urgent or sick visits, or are considered medically necessary by us. Exam fees do not include the cost of diagnostics, procedures or products that may be administered, prescribed or recommended by us. Appointment changes or cancellations must be made at least twenty-four (24) hours before the scheduled time to avoid any late cancellation or missed appointment fees, as further described in the Chewy Vet Care Terms of Use.

If you receive discounts on Chewy.com because of your employment status, the Flea, Tick and Heartworm discount of the Plan cannot be added to the employee discount for those purchases.

PAYMENT

By providing your debit or credit card, you expressly agree we are authorized and will charge your debit or credit card for any payments in accordance with the Plan selected by you (the “Payments”). The first Payment will be charged on the Plan’s Start Date and will be charged monthly for monthly Plans and charged annually for annual Plans (where available), until the Plan is terminated in accordance with these Terms. Payments may only be paid using debit or credit cards. Prepaid cards and gift cards may not be used to make Payments.

We will charge Payments using the payment processing vendor selected by us. Payments for Plans can be made on a monthly basis or annual basis, depending on the plan available at your Chewy Vet Care location and which payment option you select. You are responsible for keeping your payment information up to date. If payment information changes, you must notify us via email (careteam@vetcare.chewy.com) or phone (877-372-4399) to ensure the Plan remains active. If we are unable to charge and collect the Payments when they are due, the services and benefits available under the Plan will be suspended until you make all Payments due. We may cancel the Plan for nonpayment of any charges. Payments are inclusive of sales tax and all other relevant taxes and charges.

PLAN TERM AND AUTORENEWALS

The Plan starts on the day you make the first Payment to us ("Start Date"). The Plan lasts for twelve (12) months from the Start Date and will automatically renew each year on the same date unless you or we cancel it as described in the Terms. The Plan covering your Pet will automatically renew, unless you request a change before the Renewal Date. We will send your email address on file a notification about any changes to the Plan, including Payments, pricing, services and benefits, at least thirty (30) days in advance of the Renewal Date.

We may discontinue the Plan or terminate the Plan for any reason and at any time and will notify you in writing of those changes.

PLAN CANCELLATION

We may cancel the Plan for nonpayment of any charges. If we cancel your Plan for this reason, you may become ineligible for enrollment in any other Plan in the future or you may be required to make all Payments in advance for the full twelve (12) month term before a Plan may be reopened, as well as any Payments owed under the prior Plan. We may also cancel a Plan at our sole discretion at any time. Plan cancellations will be effective as soon as we inform you of the cancellation.

You may cancel the Plan at any time by notifying us in person, over the phone at 877-372-4399, or in writing careteam@vetcare.chewy.com For the cancellation to be effective, you must provide us with your name, the Pet’s name, your email used during enrollment along with the requested cancellation effective date and the best phone number to reach you

If you cancel the Plan after the Start Date or annual renewal, and your Pet has not received any Plan Benefits, we will refund all Payments paid by you for that Plan. If you cancel the Plan after the annual renewal or after your Pet has received Plan Benefits, you will be responsible for the difference between the value of all Plan Benefits received and the amount paid by you for all such Plan Benefits. We will charge any amounts owed using your payment information saved to your account at the time of cancellation. If we are unable to complete your Payment with the payment method you provided to open the Plan, we may charge any other valid payment method associated with your account, or we may notify you and give you the opportunity to add another payment method in your account. You must cancel your Plan before it renews in order to avoid billing of the Payments for the next billing cycle.

NOTIFICATIONS

You agree that, in order for us to service your account, including, but not limited to, providing medical reminders, product recalls, or to collect any amounts owed, that we may contact you by telephone, and/or emails, at any telephone number or any email address provided by you to us.

LATE CANCELLATIONS & MISSED APPOINTMENTS

 Your appointment may be subject to Chewy Vet Care’s cancellation policy, as stated in the Chewy Vet Care Terms of Use.

THE PLAN IS NOT INSURANCE

The Plan is not insurance and does not replace insurance.

MODIFICATIONS

We may in our discretion change these Terms, the Benefits offered under the Plans, or any other aspect of the Plans, without notice to you. If any change to these Terms is found invalid, void, or for any reason unenforceable, that change is severable and does not affect the validity and enforceability of any remaining changes or conditions. If you do not agree to any changes, you must cancel your Plan. Your continued membership in the Plan after we change these Terms constitutes your acceptance of the changes.
