The amount given depends on the weight of your dog. Dogs weighing between 26-30 pound need 1 tablet, 31 - 44 need 1.5 tablets, and 45 - 60 needs 2 tablets.
How do you know the treatment worked completely k

The treatment is working if you notice the worms in the dog's stool and the stomach swelling going down. We'd recommend consulting with your vet to do a final test to check all the worms are gone.
Does this single treatment kill eggs to?

This de-wormer only kills worms and would not kill the eggs or larvae.

Steps should be taken to control fleas and rodents on the premises in order to prevent reinfection; otherwise, retreatment will be necessary. This is especially true in cases of tapeworms transmitted by fleas (Dipylidium caninum) where reinfection is almost certain to occur if you do not remove fleas from your pet and its environment. If reinfection occurs, tapeworm segments may be observed within one month of the initial treatment. As a minimum adult dogs should be treated every three months for intestinal worms. Many monthly heartworm and flea treatment products also treat for worms, but be sure to check that the product that you are using covers for tapeworms or you are using a product like this one.
