Chewy, Inc. and its affiliates and subsidiaries (“Chewy” or “we”, “our”, “us”) are providing this Supplemental Privacy Notice for residents in Kansas and Ohio only (“Participants” or “you”) who participate in the Controlled Substance Prescription Pilot (“Supplemental Privacy Notice”). We describe below the source of where we collect the personal information, the categories of personal information we process, the reason why we process personal information, to whom we may disclose this information to, and the criteria we use for retaining this personal information, all in connection with the Controlled Substance Prescription Pilot program.

SOURCE OF PERSONAL INFORMATION
As part of the Controlled Substance Prescription Pilot, we collect personal information from different sources. For example, we may collect personal information directly from Participants when they create an account, request prescriptions, place an order, interact with us or otherwise provide us personal information. We may also automatically collect personal information from a Participant’s device or Internet Protocol (“IP”) address and based on the Participant’s Internet or other electronic activity information when they use our websites or applications. Further, we may collect personal information through the ordering systems we use, Participant’s transaction history, the prescription process and third parties who provide us information as part of the Controlled Substance Prescription Pilot, such as veterinarians, pharmacists, Prescription Drug Monitoring Programs (PDMPs), and service providers that support identity verification and prescription processing. Pharmacists may also access your information contained in the Prescription Drug Monitoring Program (“PDMP”) electronic database.
INFORMATION WE OBTAIN
We obtain different categories of personal information about Participants as part of the Controlled Substance Prescription Pilot. The categories of personal information are: (1) contact and other information about the Participant, such as name, address, email address, phone number and date of birth; (2) communications information, such as call recordings, chat transcripts and email communications; (3) account information, such as the Participant’s account login credentials; (4) persistent identifiers and Internet or other electronic activity information, such as IP addresses, device IDs, browsing and search history, website and application usage, and other information processed through cookies and other trackers; (5) the approximate location of the Participant; (6) order information and data contained in the PDMP electronic database, such as prescription and health data and purchase history; and (7) payment information, such as credit card number, debit card number, bank account and other financial information.
HOW WE USE THE INFORMATION WE OBTAIN
We use the personal information we obtain about Participants for a variety of purposes, including to: (1) fulfill the orders and process prescriptions; (2) process payments for the orders; (3) create and manage Participant’s account; (4) verify identity, prevent fraud and security issues, and address legal and regulatory compliance, including requirements under the PDMP and other controlled substance laws; (5) maintain records pursuant to our retention schedule and other business and legal requirements; (6) provide customer support and quality assurance; and (7) with respect to website and application usage data, to ensure platform functionality, obtain analytics and use for marketing purposes (consistent with the Chewy Notice at Collection and Privacy Policy).
INFORMATION WE DISCLOSE
As part of the Controlled Substance Prescription Pilot program, we may disclose personal information to the following categories of recipients: (1) service providers and processors, such as those who provide identity verification, cloud, data hosting, IT, analytics, security, customer support, communication platforms, payment processing, logistics, and marketing services; (2) those involved in the pet’s healthcare and prescription order, such as veterinarians, pharmacists and the PDMP systems; (3) our affiliates and subsidiaries; and (4) to other third parties consistent with the “Business Transfer” and “Protection of Chewy and Others” provisions in the Information We Disclose section of the Chewy Notice at Collection and Privacy Policy.
DATA RETENTION
We retain the personal information processed under this Supplemental Privacy Notice for the time period reasonably necessary for the Controlled Substance Prescription Pilot, managing your account while it is open, legal, regulatory, recordkeeping and financial obligations, and pursuant to our internal retention schedule and policies.
CHANGES TO THIS POLICY
We may change this Supplemental Privacy Notice from time to time. If we make any changes, we will change the “Last Updated” date above and post the new Supplemental Privacy Notice here. We encourage you to review this Supplemental Privacy Notice, along with the Chewy Notice at Collection and Privacy Policy, regularly for any changes. Where required by law, we will provide notice of material changes. We will notify you by email (sent to the email address specified in your account) or by means of posting a notice on this website prior to the effective date of the change.
